LifeCare Memory Partners named participant in Medicare/Medicaid GUIDE Program
We have some extremely exciting news! Lifecare Memory Partners has been selected and will be participating in the Center for Medicare and Medicaid (CMS) GUIDE Model. This is a significant achievement for our program. To be chosen, our organization went through a competitive application process and our acceptance is a testament to the incredible work we’ve been doing with our LifeCare Memory Partners program. Fewer than 20 programs in NC and SC were chosen to participate in the GUIDE program. Our dedication to providing comprehensive patient and caregiver support has not gone unnoticed, and now we have the opportunity to extend our impact far into the future.
The GUIDE Model is an eight-year demonstration model designed to offer robust support structures for patients and caregivers, with the aim of enabling patients to remain in their homes for longer periods while alleviating the burdens faced by caregivers. Our acceptance and participation in this program speak volumes about the expertise and dedication of our LifeCare Memory Partners team.
Officially launching on July 1, 2024, the CMS GUIDE program marks a new chapter for us. Initially, we will be focusing our efforts on New Hanover and Brunswick counties, leveraging our strong presence and experience in these areas. This strategic approach will enable us to ensure a smooth transition as we adapt to the requirements of the new program. Looking ahead our goal is to expand LifeCare Memory Partner services into adjacent NC counties and eventually into South Carolina during the next few years. In addition to expanding our LifeCare Memory Partners service footprint under the GUIDE model, we will, of course, continue to support LifeCare Memory Partners participants who are not eligible for GUIDE as set forth by CMS.
