After our initial consultation, we will develop a personalized plan of care that includes regular follow up visits and encourages both the caregiver and patient to attend our programs. This program also provides access to increased levels of support when appropriate, including home care, palliative care, and hospice care.

  • Whether you are new to the challenges of living with dementia or have years of experience, we encourage you to join our community to share your experience and learn from others. You are not alone in this journey.

• Empowering caregivers to navigate the challenges of dementia. It is vital that caregivers receive the support they need to alleviate caregiver stress and burnout.
• We offer a number of options to support caregivers:
  
  • Respite vouchers: We will reimburse eligible caregiver respite expenses (adult sitter, home aide, etc.) for qualifying individuals. Please contact us for details
  • Volunteer respite: Have one of our trained volunteers accompany your loved one while you tend to your own health, wellbeing, or get some errands done.
  • Group Activities:  We offer a variety of activities during our caregiver education sessions where we will look after your loved one in a secure setting.

• Caregiver Training: Join us in our state of the art simulation lab where we can show you the proper techniques to maneuver your loved one. 
• Community Education: We offer in person and virtual education programs for local businesses, community organizations, first responders, and more.
• Clinician training: Complete your continuing education requirements with us. We offer continuing nursing education for registered nurses (RN’s), licensed practical nurses (LPN’s), and social workers. We also provide Certificates of Attendance for Medical Technicians, Nurse Aides, and Medical Assistants (MA’s).

• Caregiver Connections: Facilitated by our team, this support group for caregivers is offered in person and virtually.  We start each meeting with 10-15 minutes of education and then open the meeting up for informational sharing.
• Good Vibrations: This is an in-person engagement group for people in the early stages of dementia who are living alone or living with a caregiver. Caregivers are encouraged and welcomed to attend. The focus is socialization with others while enjoying a group activity.  We always have fun!
• Expressive Arts: This is an in-person two-hour group offered to our Memory Partner clients during some of our in-person Savvy Caregiver classes for caregivers being provided at our 1414 Physicians Drive Wilmington NC 28401 location.

We are proud to work with our local, state, and national Dementia Partners. Together, we connect families to valuable resources and collaborate on fundraising events to increase awareness about Alzheimer’s Disease and related dementias.

Knowledge is power – especially when caring for a loved one with a dementia-related illness. Understanding how the illness advances and affects the brain is key to being able to communicate with you loved one in a positive way.
